Cited Form Drafts for Clinician Sign-off
Codex drafts administrative forms from what your clinic has already documented. Every value it enters is cited to its source. Where the record doesn’t answer a question, the field stays blank. You review and edit the draft, publish it, and sign the form yourself.
Starting with the Sun Life attending physician statement, the CRA T2201 Disability Tax Credit certificate, and the BC PPMB application.
1. A form is requested.
The patient consents, and a short questionnaire covers what the chart doesn’t.
2. Codex drafts it.
Every value is cited to its source: a clinic document, or the patient’s own answers.
3. You review.
Check any value against its source and change what you need to. Fields that call for clinical judgment are always yours to complete.
4. You publish and sign.
Nothing is released from Codex until you publish it. You sign the form yourself, and you decide where it goes.
What Codex does not do
Codex drafts administrative forms. It does not diagnose, treat or evaluate patients, and it never completes fields that call for clinical judgment. It does not connect to or write to your EMR: in the pilot, it works only with documents your clinic uploads, for patients who have consented. It does not send forms to insurers or anyone else. That stays with you.
